			Does the plan address how the organization will respond to any allegation of non-compliance?
			Does the plan address what procedures will be followed if a violation has been detected?
			Does the organization have a signed acknowledgement of receipt, review and understanding of the billing and reimbursement compliance plan from every individual, including outside vendors that are affected by the plan?

CLIA Compliance

			Does the organization have a current, written Laboratory Procedure Manual? (PPM and above) If Yes, has it been updated to reflect the 2004 modifications in the regulations?

			Does the organization conduct annual laboratory training?
			Does the organization maintain and retain training records?
			Does the organization only perform tests requested either in writing or electronically?
			Does the organization maintain these requests for at least two years?

HIPAA
